Forms – Step 1 – Create new form

Overview

This section describes the steps to create a form using Form Designer. Form creation is a three step process.

  1. Create a form with the basic attributes like the form name, form code, language, form description and where the form is used.
  2. Use the Form designer to define the “Header, Footer, sections and questions”. The form is in draft state.
  3. After review, publish the form for use.

This section describes the steps the create a form with basic attributes.

  • Form title (required)
  • Form Code (required)
  • Where this form is used (related to appraisal, candidate evaluation, development plan etc) (required)
  • Language – default is English
  • Form Description (optional)
  • Belongs to any company/Department (optional)
  • Scoring enabled – Default is yes.¬†Forms can be used for data capture (or) for evaluation. Forms used for evaluation of candidates , employees etc… should have scoring enabled.
Training
  • Sign-in to Athri as user with L3 privileges and above.
  • Click “Manage” link from the top menu.
  • Click “Form Designer” link.
  • The list of existing forms screen appear.
  • Click ‘New Form” button.
  • Create Form screen appears.
  • Enter the details.
  • Click “Create” button to create a skill record.
  • Form is created and the form is in “Not Ready” state.
